On March 24, 2004, the Academic Senate approved a form as an addition to the new program approval process. This Financial Implication Form, (or Word), verifies that the financial implications of any new program have been examined and that resources are available for the implementation of the program proposed. The completed form (with all approval signatures) must be uploaded with the new program curriculum proposal in the Curriculum Forms System.
Request to Add Hours to a Program
The Request to Add Hours Form is required to be uploaded with any program revision proposal that will increase total hours required in an existing undergraduate or graduate program (degrees, sequences, minors, and certificates).
Course Materials Fee Request
The Provost's Office coordinates theMaterial Fee processes. These requests and approvals are not part of the curriculum process.
