Financial Implication Form
To complete the curricular approval process, proposed new undergraduate and graduate programs must include information concerning what financial resources are needed to begin and sustain the program. Refer to the Academic Planning section of the Provost’s website for information about the required Financial Implication Form before you begin the online proposal process.
Request to Add Hours to a Program
The Request to Add Hours Form is required to be uploaded with any program revision proposal that will increase total hours required in an existing undergraduate or graduate program (degrees, sequences, minors, and certificates).
Course Materials Fee Request
The Provost's Office coordinates the Material Fee processes. These requests and approvals are not part of the curriculum process.
